Information and Assistance (I&A)
I&A staff assist seniors by linking them to available resources
including personal care services, counseling, employment, family caregiver
support, financial assistance, legal services, nutrition services, prescription
assistance, respite care and transportation. I&A staff also provide
general information, support, and advocacy. Prescription assistance
access services are also available to any adult over the age of 18.
This service is free of charge.
I&A
staff are also responsible for outreach and educating the community
about LMTAAA services and to network with
other providers
to maintain an updated resource directory for clients and staff.

Case Management
LMTAAA Case Managers assess the temporary and permanent needs of elderly
and younger persons with disabilities to coordinate services in order
to maintain maximum independence. Case Managers assess needs, develop
care plans, authorize and arrange services, coordinate provision of
services among providers, and follow up with re-assessments as needed.
Family Caregiver Support Program (FCSP)
LMTAAA staff plan, coordinate and provide services and support for unpaid family caregivers of adults and children who live with chronic illness or disabilities. Services include information and assistance, training, respite care, and other services to help support the family caregiver in providing care to a loved one. Each LMTAAA office has a caregiver resource library with books, videos, and periodicals available for loan. Free classes on caregiving and coping skills are offered periodically throughout the year in all three counties.

Community-Based Services
Adult Day Care
Adult day care programs are community-based and designed to meet the
needs of adults with impairments through individual plans of care. These
types of structured, comprehensive, non-residential programs provide
a variety of health, social and related support services in a protective
setting. By supporting families and caregivers, an Adult Day Care program
enables the person to live in the community. LMTAAA contracts with Senior
Services for South Sound to provide Adult Day Care services in Thurston

Congregate Nutrition
(group meals)
Senior nutrition programs consist of both congregate and home-delivered
nutrition services to help increase the nutrient intake of older individuals
who might not eat adequately, and, through better nutrition, assist
them to remain healthy and independent in their communities. Congregate
nutrition services are served to groups of seniors in a variety of settings
across our region and allow participants to form new friendships and
to interact in a social environment. LMTAAA contracts with two organizations

Kinship Care Programs
With funding from the National Family Caregiver Support Program and
the State Kinship Caregivers Support Program, LMTAAA contracts with
Family Education and Support Services, a program of Child Care and Educational
Services,
to provide services to grandparents and other relatives raising children
in all three counties. Services include support groups with concurrent
children's programs and childcare, Kinship Navigator services,
periodic intergenerational activities, and emergency support services.

Legal Services
Legal assistance services provide access to the system of justice by
offering advice from or representation by a legal provider who acts
as an advocate for the socially and economically needy older individual
experiencing legal problems with priority issues that are established
to reflect local needs. LMTAAA contracts with Northwest Justice Project
to provide legal services in Lewis, Mason and Thurston Counties. For

Long Term Care Ombudsman
Program
The Ombudsman Program is a volunteer-based program intended to improve
the quality of life for people who live in licensed long-term care facilities.
Residents in long-term care facilities are guaranteed certain rights
by federal and state laws and regulations. The purpose of the program
is to protect and promote these rights in long-term care facilities
and help residents to become self-advocates.

Senior Farmer's Market Nutrition Program
(SFMNP)
The SFMNP provides low-income seniors with $40 in vouchers to purchase
fresh, nutritious, unprepared, locally grown fruits, vegetables, and
herbs from authorized farmers' markets. Additional resources in the form
of nutrition educational materials including recipes, nutrient values,
food preparation and storage information are also provided. For more

Eligibility: age 60+ (or age 55+ if American Indian or Alaska Native) and at or below 185% Federal Poverty Level ($1,670/mo single or $2,246/mo couple).
Eligible persons are provided with $40 of checks, redeemable for the purchase of fresh fruits and vegetables at authorized Farmers Markets and/or Roadside Stands.
Authorized Farmers Markets within our region are in Olympia, Lacey, Tumwater, Shelton Centralia and Chehalis. Spooners Berry Farms will have authorized Roadside Stands in Lewis and Mason Counties.
SFMNP checks will be distributed on a first come/first serve basis at senior centers, food banks, low income housing complexes and other sites in all three counties (Lewis, Mason, Thurston).
Distribution of SFMNP checks will occur in late May/early June and the checks are redeemable from June thru October 2010.
Check back around mid-May for the posting of the finalized distribution schedule for the 2010 season.

Transportation
Transportation services are designed to transport older persons to and from medical and health care services, adult day services, essential errands, social services and senior nutrition program meal sites so that such services will be accessible to eligible individuals who have no other means of transportation. LMTAAA contracts with three organizations to provide transportation services in our region.

In-Home Services
Home Delivered Meals
Senior nutrition programs consist of both congregate (group meals) and
home- delivered nutrition services to help increase the nutrient intake
of older individuals who might not eat adequately, and, through better

Home-delivered meals are provided to vulnerable older persons who are
normally unable to leave their homes without assistance. LMTAAA contracts

Personal Care Services
Personal Care Services are provided by contracted Home Care Agencies
and Individual Providers. Personal Care Services are services that assist
an individual to stay in his/her home and live as independently as possible.
Services may include assistance with eating, toileting, ambulation,
transferring, positioning, bathing and self-medication. Services may
also include adaptations to the home, Personal Emergency Response Systems,
and skilled nursing care and training.

Respite Care
Respite Care relieves family caregivers of caregiving duties for short
time periods. Respite Care can include personal care or supervision.
Respite Care is provided in the home by contracted home care agencies,
or out of the home by contracted providers such as home care agencies,
nursing facilities or adult day care. For more information, contact the
