The Hospital Transitions Care Coordinator develops care plans for patients being discharged from the hospital to help with their return to the community. The position works closely with patients, families, caregivers, and service providers to determine the resources available to meet the patient’s needs. The position is supervised by the Access Services Supervisor.
This position is new and successful candidate will help build and design the program.
